Tapentadol 50 mg / 100 mg tablets
Tapentadol is a strong opioid painkiller for short-term use in adults when pain is severe enough to need an opioid and other treatments are not enough. It is a controlled substance: it can cause addiction, dependence and life-threatening slow breathing, even when taken exactly as prescribed. It is dispensed only against a valid prescription and after a pharmacist has reviewed your order.
This medicine is dispensed only against a valid prescription. Don’t have one? Order anyway — one of our doctors will call you for a free consultation after you order, and the medicine ships only if it is prescribed for you.

Medicine information
Tapentadol uses
Tapentadol immediate-release tablets are licensed for one situation, and the label is deliberately narrow about it.
- Management of acute pain severe enough to require an opioid painkiller, and for which other treatments are not adequate, in adults
The label limits tapentadol to patients for whom alternatives are ineffective, not tolerated, or otherwise inadequate. Your prescriber decides whether that applies to you; follow their instructions.
How to use tapentadol
Take tapentadol exactly as prescribed - the lowest dose that controls your pain, for the shortest time. Never take more, more often, or for longer than your prescriber directed, and never share it with anyone.
- Swallow the tablet whole with water. Do not crush, chew, split or dissolve it.
- You can take it with or without food.
- The label gives a usual adult range of 50 mg to 100 mg every 4 to 6 hours as needed. Your prescriber sets your dose and how often you take it.
- Do not drink alcohol while taking tapentadol, in any amount. The combination can cause profound sedation, dangerously slow breathing, coma and death.
- Do not drive, cycle or use machinery until you know how tapentadol affects you. It commonly causes drowsiness and dizziness.
- If you have taken it regularly for more than a few days, do not stop suddenly. Your prescriber will reduce the dose gradually to avoid withdrawal.
- Constipation is very common with opioids. Ask your prescriber or our pharmacist about managing it before it becomes a problem.
- Keep tapentadol locked away and out of sight. A single tablet can be fatal to a child or to anyone not used to opioids.
- Return unused tablets to a pharmacy for disposal. Do not keep leftovers for later and do not pass them to anyone else - that is both dangerous and illegal.
If you miss a dose
If you take tapentadol only when you need it, simply take the next dose when your pain requires it and your dosing interval allows. If you take it on a fixed schedule, take the missed dose when you remember unless the next one is nearly due - then skip it. Never take two doses together.
Storage
Store below 25 degrees C in a dry place, in the original pack. Keep it locked away, out of the sight and reach of children and anyone else in the household.
Tapentadol side effects
Side effects are common with tapentadol, particularly when you start or when the dose goes up; tell your prescriber or our pharmacist about anything troubling you, and treat breathing problems as an emergency.
Common
Serious — seek urgent care
- Slow, shallow or stopped breathing, extreme sleepiness, being difficult to wake, or blue-tinged lips or fingertips - call emergency services immediately
- Serotonin syndrome: agitation, hallucinations, fever, fast heartbeat, muscle twitching or stiffness, heavy sweating, sickness or diarrhoea - seek urgent care
- A seizure or fit - seek urgent care
- Signs of adrenal insufficiency after longer use: persistent tiredness, nausea, vomiting, dizziness or low blood pressure - contact your prescriber promptly
- Fainting or severe dizziness on standing, from a marked drop in blood pressure - seek urgent care
- A severe allergic reaction: swelling of the face, lips or throat, hives, or difficulty breathing - call emergency services
Warnings
- Tapentadol is a controlled substance - Schedule II in the United States and a controlled drug in Canada and most other countries. It carries a risk of addiction, abuse and misuse that can lead to overdose and death, and that risk exists even when it is prescribed appropriately.
- Life-threatening respiratory depression can occur at any time, but the risk is highest when you first start and after any dose increase. Anyone in your household should know to call emergency services if you become very drowsy and hard to rouse.
- Do not combine tapentadol with benzodiazepines, sleeping tablets, gabapentin or pregabalin, other opioids, or alcohol unless your prescriber has specifically weighed it up. Together they can cause profound sedation, respiratory depression, coma and death.
- Do not take tapentadol if you use a monoamine oxidase inhibitor (MAOI) or have taken one in the past 14 days.
- Do not take tapentadol if you have significant breathing difficulty, acute or severe asthma without monitoring, or a known or suspected blockage of the bowel including paralytic ileus.
- Physical dependence develops with regular use. Stopping suddenly can cause withdrawal - restlessness, watering eyes, sweating, chills, aching, sickness and diarrhoea. Your prescriber will taper the dose.
- Prolonged use in pregnancy can cause neonatal opioid withdrawal syndrome in a newborn, which can be life-threatening. Tell your prescriber if you are pregnant, might be, or are planning a pregnancy.
- Accidental ingestion of even one tablet, particularly by a child, can cause a fatal overdose. Store it locked away.
- Serotonin syndrome can occur with SSRIs, SNRIs, tricyclic antidepressants, triptans, tramadol, linezolid or St John's wort. List everything you take, including anything bought without a prescription.
- Extra caution is needed if you are elderly, frail, have chronic lung disease, a head injury or raised pressure in the head, a seizure disorder, or liver or kidney impairment.
- Never give tapentadol to anyone else. Sharing a controlled medicine is illegal and can kill someone who is not used to opioids.
Tapentadol FAQs
How long does tapentadol take to work?
Immediate-release tapentadol is designed for fast relief of acute pain, and most people notice an effect within about an hour of a dose. The label allows a second dose one hour after the first if relief is inadequate, but only where your prescriber has told you that applies to you - do not decide that yourself. If the dose your prescriber set is not controlling your pain, contact them rather than taking more. Taking extra opioid raises the risk of dangerously slow breathing without reliably improving pain.
Is tapentadol stronger than tramadol?
Tapentadol and tramadol are different medicines, and milligram-for-milligram tapentadol is the more potent of the two - which is why the doses on their labels differ. That is not the same as one being the right choice for you. They differ in how they are broken down by the body, in which drug interactions matter, and in their side effect patterns. Both carry opioid risks. Which is appropriate depends on your pain, your other medicines and your health history, and that is your prescriber's decision. We dispense what your prescription specifies.
Can I drink alcohol while taking tapentadol?
No. Do not drink any alcohol while you are taking tapentadol. Alcohol adds to the sedative effect of the opioid and can cause profound drowsiness, dangerously slow or stopped breathing, coma and death - and it can do so at doses of each that would be tolerated alone. The same warning applies to benzodiazepines, sleeping tablets, gabapentin and pregabalin. If you take tapentadol, tell your prescriber about everything else you take, including anything recreational, so they can judge the risk honestly.
Is tapentadol addictive?
Tapentadol is an opioid and a controlled substance, and yes - it carries a real risk of addiction, abuse and misuse, including in people who take it exactly as prescribed. Separately, anyone who takes it regularly develops physical dependence, which means stopping suddenly causes withdrawal symptoms. Dependence is expected and is not the same thing as addiction, but both are reasons the label limits tapentadol to short-term use at the lowest effective dose. Talk to your prescriber about a plan for stopping before you start, not after.
Can I drive while taking tapentadol?
Not until you know how it affects you, and in many places not at all while you are taking it. Tapentadol commonly causes drowsiness, dizziness and slowed reactions, and those effects are strongest when you start and after any dose increase. Driving impaired is an offence in most jurisdictions regardless of whether the medicine was prescribed. Ask your prescriber what applies where you live, avoid driving entirely if you have also taken anything sedating, and never drive after drinking alcohol on this medicine.
Can I buy tapentadol without a prescription?
No. Tapentadol is a controlled prescription-only medicine, and we cannot dispense it without a valid prescription from your licensed prescriber. Anyone offering it without one is operating outside the law and outside any safety check. The prescription requirement is what makes someone responsible for checking your dose, your other medicines and your risk of respiratory depression before you take the first tablet. Send us a valid prescription and our pharmacist reviews it, and will contact you or your prescriber if anything needs clarifying.
What strengths and pack sizes do you dispense, and what does it cost?
We dispense tapentadol as 50 mg (10 tabs) and 100 mg (10 tabs). The current price for each pack is shown on this page and again in your cart before you pay, with no membership fee and no tiers. Stock may be branded or generic on any given day, depending on what is in supply; the active ingredient, the strength and the pack size are what the label says either way. What we dispense is set by your prescription rather than by what you select, so if the quantity on your prescription differs from the pack you picked, our pharmacist will contact you before anything is sent.
Where do you ship tapentadol?
We dispense only where we are licensed to, and the checkout tells you whether we can ship to your address before you pay. Controlled medicines are more tightly restricted than ordinary prescriptions, and many countries prohibit receiving them by post from abroad entirely. If we cannot dispense to your address, nothing is charged. If you are unsure, contact us before ordering and we will tell you plainly whether your address falls inside our service area.
